Senior primary care company opens 1st S.C. clinic

Oak Street Health, a primary care center network for adults on Medicare, moved into its new location at 6119 White Horse Road in Greenville on March 31.

As the company’s first S.C. location, the facility will offer both in-person and telehealth services, patient transport to and from appointments, 24/7 patient support line, preventative care plans and future community programming for Greenville County’s senior residents, which make up 16% of the population, according to a news release.

“We are excited to expand access to our high-quality, value-based care to members of the Greenville community with the opening of this new center,” Dr. Dillon Miller, senior medical director at Oak Street Health, said in the news release. ​“The majority of older adults we care for have two or more chronic conditions, making this kind of care so important. As our organization continues its mission to rebuild health care as it should be, we look forward to improving patient outcomes in South Carolina and helping older adults live their healthiest lives.”

Since its founding in 2012, Oak Street Health has reduced patient hospital admissions by about 51% compared to Medicare benchmarks, according to the release, as well as a 42% reduction in 30-day readmission rates and a 51% reduction in emergency department visits. South Carolina is the 13th state where Oak Street Health has a presence.

The company is publically traded on the New York Stock Exchange.
